Injury Release Form

I hereby declare that my child/athlete is in good health, has no known medical conditions that could be adversely affected by participation in athletic activities, and has been cleared by a healthcare professional for such activities.

Acknowledgment of Risks: I understand and acknowledge that participation in athletic activities, including but not limited to basketball training, skills development, participation in tournaments, involve inherent risks of injury. I willingly assume these risks for for my child/athlete.

Release of Liability: In consideration of being allowed to participate in activities organized by The Super Meteors Corporation, I hereby release, waive, discharge, and covenant not to sue The Super Meteors Corporation, its officers, agents, or employees (hereinafter referred to as "releases") from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me/my child, or to any property belonging to me/my child, while participating in such activities or while on premises where the activities are conducted.

Emergency Medical Treatment: In the event of an emergency, I authorize The Super Meteors Corporation to secure from any accredited hospital, clinic, and/or physician any treatment deemed necessary for my/my child’s immediate care. I agree that I will be fully responsible for payment of any and all medical services rendered.

Acknowledgment: I have read this release of liability and assumption of risk agreement, fully understand its terms, and understand that I am giving up substantial rights, including my/my child's right to sue. I acknowledge that I am signing the agreement freely and voluntarily, and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.

SPORTS CONDUCT AGREEMENT

This Sports Conduct Agreement is entered into by and between The Super Meteors Corporation ("The Super Meteors") and the undersigned athlete and their parent/guardian. This agreement sets forth the conduct expected of athletes and parents/guardians participating in The Super Meteors programs.

1. Athlete's Conduct: As an athlete of The Super Meteors, I agree to:

Demonstrate sportsmanship, respect for all participants, coaches, and officials at all times.
Attend all scheduled practices and games, unless excused for valid reasons.
Follow the instructions and decisions of the coaching staff and officials.
Refrain from any form of bullying, harassment, or discriminatory behavior.
Avoid the use of foul language, abusive behavior, and any form of physical violence.
2. Parent/Guardian Conduct: As a parent/guardian of an athlete of The Super Meteors, I agree to:

Encourage my child to play by the rules and respect the rights of other players, coaches, and officials.
Not engage in unsportsmanlike conduct with any coach, player, parent, participant, official, or any other attendee.
Not encourage any behaviors or practices that would endanger the health and wellbeing of the athletes.
Teach my child that doing their best is more important than winning.
3. Removal from Program: I understand and agree that The Super Meteors reserves the right to remove my child/myself from the program at any time, without refund, for any of the following reasons:

Breach of this Sports Conduct Agreement.
Any behavior deemed by The Super Meteors to be harmful or contrary to the best interests of the program and its participants.
4. Acknowledgment: By signing this agreement, I acknowledge that I have read, understood, and agree to adhere to the rules and regulations as set forth in this Sports Conduct Agreement. I understand that failure to adhere to these guidelines may result in disciplinary action, up to and including removal from the program without refund.
