This form allows us to collect information about the kind of support you are looking to receive. By getting to know you and your needs, we can make sure our support meets your expectations.

Will you keep my information private?

Yes, any information we receive about you will be stored privately and securely. The information you have provided in this form will not be kept for more than 7 days, unless you sign our service agreement.

If you sign our service agreement, we will store your information in your personal file in our digital system. We will keep this information secure and private, in line with our policies.

If you don't want to receive support from us, we will delete the information contained in this form. You need to tell us if you want us to delete your information earlier.
