    Section 1: Obsessions

Obsessions are unwanted and persistent thoughts, images, or urges that pop into your child’s mind and cause significant anxiety or distress. These thoughts can be about many different things and might not make much sense to your child, but they can be hard to stop or control.

As you read through the different types of obsessions below, think about whether your child experienced any of these thoughts in the past 1 month.

Check the box next to each one that applies to your child. Answer as best as you can.

    Section 2: Compulsions

Compulsions are repetitive behaviors or mental actions that your child feels they must do to relieve the anxiety caused by your obsessions or to prevent something bad from happening. These actions can provide temporary relief, but the anxiety often comes back, leading to more compulsive behavior.

As you go through the list of compulsions below, consider whether your child has felt compelled to do any of these actions in the past 1 month.

Mark each item that applies. Answer as be as you can.
