  • WORKPLACE SUBSTANCE ABUSE POLICY ACKNOWLEDGMENT OF RECEIPT

  • I understand that as an employee of ParCou LLC. I may be subject to a pre-employment drug testing, post accident drug testing, random drug testing, reasonable cause drug testing or return to duty drug testing as described in the Workplace Substance Abuse Policy.

    I understand that if my test is positive for illegal drugs or alcohol, I will not be considered for employment, or if I am a current employee, I may be subject to disciplinary action up to and including termination, in accordance with the Workplace Substance Abuse Policy.

    I understand that I may request a copy from the lab of any test taken as part of the screening process.

    I understand that the results of these test may be used for employment decisions. I hereby authorize the laboratory to release the results to ParCou LLC. I further agree to hold ParCou LLC harmless from and waive all claims existing and future for any, and all liability (including negligence) in connection with the testing for
    drugs and/or alcohol.
    Compliance with the ParCou LLC. Workplace Substance Abuse Policy is a condition of employment. Refusal to take a required drug test, or failure of a drug test, shall result in termination.

  • I have received a copy of the ParCou. Personnel Policy Handbook. I understand I am to become familiar with the contents of the handbook as it outlines many of my responsibilities, benefits and ParCou policies. Further, I understand and agree to the following:

    The handbook describes important information about ParCou LLC. but is not all-inclusive. I understand that I should consult my supervisor or the Personnel Director regarding any questions not answered in the handbook.

    I acknowledge that this handbook is neither a contract of employment nor a legal document. I understand that employment with ParCou LLC. is on an “at-will” basis. I understand that at ParCou LLC., I have the right to terminate my employment at any time. Likewise, I realize that PraCou LLC. can terminate my employment at
    any time as well.


    ParCou LLC. retains the right to at any time and for any reason, to modify, suspend, interpret, or cancel, in whole or in part, with or without notice, any published or unpublished policies or practices. Since the information, policies, and benefits described here are necessarily subject to change, I acknowledge that revisions to the handbook may occur. I understand that revised information may supersede, modify, or eliminate existing policies. Only the President or Vice President of ParCou LLC. has the ability to adopt any revisions to the policies
    in this handbook.


    I, the undersigned, have received the handbook, and I understand that it is my responsibility to read and comply with the policies and procedures contained in this handbook and any revisions made to it. Failure to comply with company policies and procedures may result in disciplinary action ranging from reprimand to immediate dismissal.


    I have read and understand the company’s Harassment and Non-Fraternization Policies and agree as part of my employment to comply with these policies as written.


    From time to time ParCou LLC. may need to do a background check at the request of a client in order to maintain a contract. I do hereby authorize my local police department to release any information concerning any criminal record that I may have to ParCou LLC. The initial police check will be run prior to employment. This information is for the sole use of ParCou LLC.


    I do hereby volunteer, without duress, coercion, or threat, agree to submit to a search of my person, or of any personal belongings brought by me onto the job site, prior to or at any time during my employment. I do hereby release, absolve, and agree to forever hold ParCou LLC and/or its employees, and all affiliated companies, harmless from any and all claims and demands arising out of or based upon damages suffered by me as a result of these examinations
