I, the parent/legal guardian of the registrant/ participant, a minor, agree that I and the registrant/participant will abide by all the rules of the Archer Travel Service (dba Archer Sports Travel / Archer Academy), its affiliated organizations, owners, and sponsors. Recognizing the possibility of physical injury or illness associated with soccer and sports activities in general and in consideration for Archer Travel Service accepting the registrant/ participant for its activities, I hereby release, discharge and/or otherwise indemnify Archer Travel Service, its owners, its affiliated organizations and sponsors, their employees and associated personal, including the owners of the fields and facilities utilized for the activities against any claim by or on behalf of the registrant/ participant as a result of the registrants/ participants participation in the soccer program and/or training programs.

WAIVER/RELEASE FOR COMMUNICABLE DISEASES INCLUDINGCOVID-19 ASSUMPTION OF RISK / WAIVER OF LIABILITY /INDEMNIFICATION AGREEMENT
In consideration of being allowed to participate in athletic programming, sanctioned activities and events (collectively “Sanctioned Activities”) held by or related to Archer Travel Service (dba Archer Sports Travel / Archer Academy) or any association to which it belongs, including but not limited to the any affiliate members (“RELEASEES”), the undersigned acknowledges, appreciates, and agrees that: 1. Participation in such Sanctioned Activities inherently includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and, 2. I KNOWINGLY AND FREELY ASSUME ALL SUCH INHERENT RISKS, both known and unknown, EVEN IF ARISING FROM THE ACTIVE OR PASSIVE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and, 3. I willingly agree to comply with the stated and customary terms and conditions for participation in such Sanctioned Activities as regards to protection against infectious diseases. If, however, I observe any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and, 4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Releasees their officers, directors, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the Sanctioned Activities , WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE ACTIVE OR PASSIVE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby agree to defend and indemnify RELEASEES for all such claims, causes of actions, allegations or matters arising out of, relating
to, based upon or in any way connected to my participation in such Sanctioned Activities.

This is to certify that I, as parent/guardian, with legal responsibility for this participant, have read and explained the provisions in this waiver/release to my child/ward including the risks of presence and participation and his/her personal responsibilities for adhering to the rules and regulations for protection against communicable/ infectious diseases, including those set forth by Archer Travel Service. Furthermore, my child/ward understands and accepts these risks and responsibilities. I, for myself, my spouse, and child/ward, do consent and agree to his/her release provided above for all the Releasees and myself, my spouse, and child/ward do release and agree to indemnify and hold harmless the Releasees for any and all liabilities incident to my minor child’s/ward’s presence or participation in these activities as provided above, even if arising from their (releasees’) negligence, to the fullest extent provided by law.

By signing this Photo, Video and Sound Recording Release and Consent Form, you are irrevocably giving permission to Archer Travel Service (dba Archer Sports Travel / Archer Sports Academy), and its’ agents to take and use photographs, video or sound recordings of your child within the understood scope of general and normal youth soccer activities, trainings and general athletic play as it relates to youth soccer play. This is completely voluntary. Your consent to the use of the photographs, video and sound recordings and your child’s image, likeness, appearance, and voice is forever. You will not receive compensation for the use of your child’s image, likeness, appearance, and voice now or in the future. Archer Travel Service may use the photographs, video and sound recording containing your child’s image, likeness, appearance and voice in any manner or media, including use on web pages. The photographs, video and sound recordings may be used in whole or in part, alone or with other recordings. The photographs, video and sound recordings may be used for any educational or informational purposes whatsoever, but not for any commercial uses. No Limits Soccer Academy has the right and may allow others to copy, edit, alter, retouch, revise and otherwise change the photographs, video and sound recordings at their discretion. All rights, title, and interest in the photographs, video and sound recordings belong solely to Archer Travel Service. You further give permission to Archer Travel Service to use your child’s name and age, and any other descriptions, events, or other material in or in connection with any such uses of the photographs, video and sound recordings. I understand and agree to the conditions outlined in this Photograph, Video and Sound Recording Release and Consent Form. As the parent or legal guardian of the participant named below and on behalf of my child, I irrevocably give consent to Archer Travel Service, agents, employees, forever to make use of my child’s image, likeness, appearance, and voice in photographs, video and sound recordings as described above. I acknowledge that I am fully aware of the contents of this release and am under no disability, duress, or undue influence at the time of my signing of this document.

REFUND INFORMATION

The cut-off for a full refund is December 15, 2024. After December 15th, you are only eligible for at 50% refund.

After February 15th, 2024, you are no longer eligible for a refund. We need to collect all money 7 days before trip starts (at latest)
