• New Student Information Form for the CAT Institute Foundation Program

    Please complete this form when you are confident you are ready to register for the first course in the CATI Foundation program, Coaching Essentials (this is not a contact form for inquiries about the program).

    You will be taken to a payment page following your submission of the form. Once you complete payment, you will automatically be registered and accepted into the program, and can expect to hear from your instructors about 3 weeks before the first class. Not all fields are required, but some are designed to help us meet your needs in the training process.
