Rationale
*
This field is required.
Catalyst Study Hall is committed to the philosophy of providing quality educational services within a Christian environment. This Student Code of Conduct has been established to assist Catalyst in fostering personal integrity and responsibility among our students. Participants are expected to meet the highest standards of personal, ethical, and moral conduct possible. Catalyst Study Hall, in its sole discretion, will make the final determination of whether there has been a violation of the program's conduct policies. Serious violations may result in exclusion from daily activities, and a request to withdraw the student from the program. If a student is asked to withdraw from the program due to a Code of Conduct violation, no refund of tuition or waiver of financial obligations will be given. Please initial below to indicate you acknowledge and agree to these terms.

Authority
*
This field is required.
Catalyst Study Hall's Board has given authority for all behavioral/disciplinary matters to the Director. In turn, the Director has delegated to the Staff the responsibility to administer discipline on a daily basis. Any person observing or having reliable information relating to a violation of the Code of Conduct may report verbally or in writing to the lead staff for investigation and further action. When warranted in the sole opinion of the staff, a search of a student's person, backpack, phone, car, athletic bag, purse, or other personal property may be conducted by Catalyst Study Hall staff. The Director is authorized to impose disciplinary sanctions, and holds the final decision on appeal. In the case of a serious offense leading to expulsion from the program, if in the opinion of the board, there is a material dispute about the facts of a case, or lack of clarity about the applicability of a policy, the Director may call for a hearing between the Academics Committee and the Board. Please initial below to indicate you acknowledge and agree to these terms.

When, in the sole opinion of Catalyst Study Hall Staff, a student is untruthful during an investigation into a possible violation of this Code of Conduct, the penalty imposed on the student will be significantly increased. Conversely, when Catalyst Study Hall Staff believes a student is cooperative, truthful, and repentant, the penalty for a violation may be significantly reduced. The opinion of Catalyst Study Hall Staff will be formed by the total cumulative information gained in the investigation.

If a student has violated or is struggling with temptations to violate this Code of Conduct, and if the student is wanting help with such issues, the student may, without fear of disciplinary action under this Code of Conduct, seek the counsel of the Director, Counselor, or any Catalyst Study Hall Staff or Board Members. Such an exemption from disciplinary action is not available if the student seeking counsel is, in the sole discretion of Catalyst Study Hall Staff, seeking to avoid the consequences of an impending investigation and possible findings of a violation. Information shared by a student will NOT be kept confidential if, in the sole opinion of Catalyst Study Hall Staff, failure to share information might result in harm to the student or other individuals. Depending on the nature of the information shared, communication with parents or civil authorities may be necessary. The nature of such information may include but is not limited to involvement with drugs or alcohol, self-harm, inappropriate sexual activity, including pregnancy, and the possibility of violence.

California statutes require the program to report violations of the law to civil authorities. When conduct policy violations occur that may also be violations of the law, Catalyst Study Hall Staff will notify civil authorities when, at the sole discretion of the program, it is determined to be required by law or otherwise determined to be appropriate. When civil authorities are involved, Catalyst is not obligated to wait on or concur with the findings of the civil authorities in determining the appropriate action under the Catalyst Study Hall policies.

The program reserves the right to consider any attempt to commit a violation of the Code of Conduct as a completed violation. Accomplices may be considered as violators and face similar consequences. We strongly recommend that students avoid situations where others are violating the Code of Conduct policies. When students find themselves in such situations, they should remove themselves immediately to avoid being implicated (2 Timothy 2:22).

Tuition Fees
*
This field is required.
Tuition is $5500.00 per year ($550 a month). The cost of our program is significantly cheaper than many programs, including one-week summer day camps. There is a $275 discount for paying the full tuition upfront or in two half-year payments; a 5% discount for every sibling enrolled after your first child is available for families paying monthly. This form is to convey which option works best for you. If you choose the monthly payments option,
please note that payments are due by the 15th of each month;
this allows time for processing on our side by the end of the month.Catalyst Study Hall offers flexible attendance;
however, once a student is enrolled in our program, full payment for each month is required, regardless of how many days your student attends
. We do not prorate our fees. If something arises where you need to withdraw your student from our program before the end of the school year, we ask that you provide us with at least 30 days notice.
$5225.00 - Full-year tuition payment, due by Aug. 15 (savings of $275)
$2612.50 - Two half-year tuition payments, due by Aug. 15th and Dec. 15th (savings of $275)
$550.00 - Monthly tuition payments, due by the 15th of each month (August through May)

Financial Commitment Signature
*
This field is required.
When you enroll your student at Catalyst Hall, the program treats tuition as a legal financial obligation. Catalyst Study Hall creates a budget and makes financial decisions based on enrollment and the timely full payments of its members.
If you are five (5) or more days late in making a payment, a charge of $75 will be added to your account. If your payment is returned due to insufficient funds or a stopped payment, pursuant to California Civil Code section 1719(a) you are liable for the full amount of the check plus a service charge of $25.00 for the first bounced check and $35.00 for each subsequent bounced check.
If your account is 30 or more days past due, your student will be immediately unenrolled from our program, until your financial commitment is met. If you are late with your payments 3 or more times, you will be ineligible for re-enrollment in our program.
If you encounter a financial hardship during the payment commitment time frame you made, please talk with office staff immediately. All information will be kept between you, the office staff, and the finance committee. No obligation is required by Catalyst Study Hall to provide discounts, scholarships, or write-offs to help families, but considerations will be made with the utmost respect and investment of prayer and family need. By signing below, you agree to the terms above, and acknowledge this constitutes a legal and binding agreement.

*
This field is required.
Catalyst Study Hall is a non-profit ministry that runs off of the hourly contributions of many people.
We have a 20-hour volunteer commitment for each family each school year that must be met. A buyout option is available for $20 an hour for partial hours or for $400 total.
Please choose whether you will meet the hourly commitment or whether you would like a buyout option invoice sent to you. Extended families can meet these hourly obligations too. Choosing either of the options is an acknowledgment that you understand these terms and obligations.
Mid-year invoices will be sent during the month of January for families who have not completed 10+ hours in the first semester. Year-end invoices will be sent during the month of May for any outstanding volunteer hours. Please note, unpaid volunteer hour invoices may result in immediate withdrawal from our program until the invoice is paid.
Volunteer opportunities will come in the form of classroom hours, office hours, event help, board or committee volunteering, yard duty, grandparent-on-campus program, dads-on-campus security, fundraising facilitation, and facility maintenance.
I (or someone in my family) will volunteer 20 hours
I would like a buyout option invoice sent to me at $20/hour
