Dent Hero Work Order

I, the undersigned, hereby authorize Dent Hero, a Division of Windshield Hero LLC, to perform services detailed in this invoice. I agree to pay the total amount due for services provided immediately upon completion, unless alternative payment arrangements have been pre-approved by Dent Hero.We strive for excellence in every repair. However, due to the nature of dent pulling and metal manipulation, there is a small possibility that the dent may not be fully repairable, or not fully go away. Additionally, in rare cases, paint may be affected, or glass could sustain damage when rods are used in the door frame. Please rest assured that we take every precaution to minimize these risks, but I now understand the risks associated with PDR.In the highly unlikely event that the above issues occur, or that you are not satisfied with the repair, our liability for this repair is limited to a refund/no charge of any amount for the attempted repair. By signing below, you acknowledge that you have been informed of these expectations, warranties, and disclaimers, and agree to release Dent Hero from any liability beyond the terms outlined in this agreement.
