If you have multiple children being photographed, you only need to fill out one form. Please list all names below, then select the photo size(s) and quantities you want in each background section you see.

Example: If you list Joe, Jane and Jack Smith, and select 3 8x10s, 3 4x6s and 1 band photo, you'll receive an 8x10, 4 4x6s for each child, and one band photo. If you want something different for each child, specify it like this: Joe Smith - 8x10, Jane Smith - 4 4x6s in the Special Instructions box below. Someone will contact you if there are any questions.

After filling out this form, please print it and bring it with your payment to the school on picture day (**make check payable to MHICKMANPHOTO LLC). Note: If this step isn't followed, your photos will be placed on the school's gallery page at mikehickmanphoto.com, and you'll need to pay for shipping.

Photos printed on professional photo paper sheet with Lustre finish, quality inks and color/exposure corrections (click box in top left corner to select and then sizes, quantity of photos and whether you would like wording printed on photo which is off by default. Big Screen & Action Figure will have wording printed on photo as displayed). You can click on the + on each photo to see it larger for examples.*
