In case of medical emergency or general medical care, I give consent for medical teatment for the aboved named camper by authorized personnel. The camp carries secondary insurance. I understand that the above named camper will only be released to the names listed above, an update may be done at registration. I certify that my child has my permission to attend camp and participate in all activites. I agree to Chesterfield MVP's Camps and Events Rules and I will read these to my children before they attend an event.

Payment Information (Please Read)

Please mail your payment out as soon as you complete our online registration. We are ONLY accepting checks for registration. After filling out this registration and hitting submit on the bottom, Please mail a check to: Chesterfield MVP, PO BOX 262, Crosswicks, NJ 08515. Without payment, your children are not fully registered for our camp. You will receive a confirmation email from ChesterfieldMVP@gmail.com once your payment is received/deposited.

Refund Policy

100% Refund Given until 30 Days BEFORE Event, 75% Refund Given until 3 Weeks BEFORE Event, 50% Refund Given until 2 Weeks BEFORE Event. NO REFUNDS will be given within a week Before the EVENT.
