    One-page letter describing a leader or volunteer who  planned and implemented a one-time event or do more than their recruited/enrolled expectations and duties.

  • Required Documentation for Outstanding Chaperone Award

    One-page letter describing a leader or volunteer who made the travel, training, or camping experience of young people a positive, life-long memory.

  • Required Documentation for Communicator Award

    One-page letter describing a leader or volunteer who developed newsletters, handbooks, social media or personal meetings to communicate with 4-H members and families.

  • Required Documentation for Outstanding County or Project Leader Award

    One-page letter describing a leader or volunteer who provided extraordinary and exemplary project-specific leadership either at the club or county level.

  • Required Documentation for Outstanding Club Organizational Leader Award

    One-page letter describing a leader or volunteer who have worked exclusively to make their club strong, vibrant and inclusive.

  • Required Documentation for Outstanding Outstanding District/State  Leader Award

    One-page letter describing a leader or volunteer who was/is involved at the District/State level and supported getting youth involved at the District/State level.

  • Required Documentation for Head Award: Head to Clearer Thinking Award

    One-page letter describing a leader or volunteer who is always thinking of new ways to facilitate educational programming, has dedicated countless hours to teaching 4-H members in Larimer County, and is constantly growing their own knowledge.

  • Required Documentation for Heart Award: Heart to Greater Loyalty Award

    One-page letter describing a leader or volunteer who digs deep to make the best better, always has a great attitude, and cares greatly about the success and longevity of the Larimer County 4-H program.

  • Required Documentation for Hands Award: Hands to Larger Service
    Award

    One-page letter describing a leader or volunteer who has dedicated countless hours to serving the youth in the Larimer County 4-H program, or has taken an active role in teaching youth the importance of serving our community, country, and world.

  • Required Documentation for Health Award: Health to Better Living Award

    One-page letter describing a leader or volunteer who is always looking out for the best interest of the youth in Larimer County, and serves as a positive role model to youth of all ages.

  • Required Documentation Congeniality Award

    One-page letter describing a member’s actions that went “above and beyond” to help others have a better 4-H year or County Fair experience

  • Required Documentation Diligence Award

    One page letter describing a member who tried very hard, but due to unusual circumstances had a difficult time.

  • Required Documentation Family and Consumer Science Award

    1-3 page letter with nominee’s name, age, past and present FCS project completed (nominee should have experience with multiple FCS project areas and some sort of leadership experience), learning experiences, how knowledge gained has been practically applied, how the member teaches others, and any supporting materials (photos etc.)

    Please Note: This can be a self-nomination or by a leader.

  • Required Documentation Community Service Award

    One page letter including name of the project, project objectives, what was one, project cost, project results, picture or any supporting materials (not to exceed 4 pages with supporting documents).

  • Required Documentation Head Award: Head to Clearer Thinking Award

    One-page letter describing a 4-H member who is always thinking of new ways to facilitate educational programming, has dedicated countless hours to teaching 4-H members in Larimer County, and is constantly growing their own knowledge.
