Declaration* This field is required.Whilst my child is in Junior Church I will be in St. Mary's church service. If I am unable to attend I will be responsible for appointing another adult to care for my child before and after Junior Church. That adult will inform the group leader either before the service or at the point of hand over during the service. We also understand that there is no guarantee of a first aider being present. During the year we may attend various Outings and Visits as a Junior Church. Full details of any trips will be advised in advance on each occasion. By clicking you give your consent for St Mary’s Junior Church to take your child on organised and pre-planned Outings and Visits. You also give your consent to any medical treatment that may be necessary in the event of an emergency.
