By Signing this Consent Form, I agree and understand the following: (Please tick)
Illness, Accident & Emergency Authority*

I understand that in case of illness, accident or emergency, the parent will be contacted as soon as possible. In the event of my child/ren requiring urgent medical attention, I authorise the care providers to seek medical assistance and agree to pay all related costs.
Cancellations*

I understand that cancellations made after 7am on the 16th September 2024, and during Vacation Care will be charged in full. We don't offer day swaps. Medical certificate - half charge. This will only apply to the child whom the certificate is issued for.
Transportation*

I understand that Craigburn OSHC will conduct a risk assessment for when children attend an excursion that will require transportation via Bus
Fees*

I understand if my account falls in arrears of more than 14 days, my account will be referred to a debt collection agency for recovery. I the account holder will be responsible for all associated debt collection charges. I understand that the Vacation Care fees are $65.00 for school based, $75.00 for Incursion and $85.00 for Excursion.
Late Collection Fees*

I understand that late collection fees per child apply from 6:15pm as per OSHC policy
Behaviour*

I understand that Craigburn OSHC and Vacation Care strive to keep Vacation Care a safe and happy place for your child/ren. Children attending Vacation Care are expected show concern and care for other children, to act with courtesy, cooperation and common sense. If children are continually not meeting our behaviour expectations they may be excluded from the program.
Excursion*

I understand that my children will be participating in supervised planned excursion as outlined in the Vac Care program and in the hover text box's above and that they must be at OSHC 30 minutes prior to excursion departure time. The bus will depart on time, failure to be here will result in your child missing the excursion and the session fee will stand.
Photograph Consent*

I consent for my child/ren to be photographed and for their image and first name to be used in OSHC & Vacation Care publicationsI do not consent
