I hereby acknowledge and agree to the following terms and conditions in exchange for participating in the science related educational tutoring program offered by Suzanne CAPA (Chinese American Parents Association of Suzanne Middle School) and Walnut High School Science Bowl and Science Olympia team members(hereinafter referred to as "the Hosts"). I understand that this program is not affiliated with Suzanne Middle School, Walnut High School, and Walnut Unified School District.
1. Assumption of Risk: I understand that participation in the tutoring program may involve certain risks, including but not limited to physical injury, illness, or property damage. I voluntarily assume all such risks.
2. Release and Waiver: By participating in the tutoring program, I hereby release Suzanne CAPA, Suzanne Middle School, Walnut High Science Bowl Team, Walnut High Science Olympia Team, Walnut High School, Walnut Unified School District, and their respective officers, directors, volunteers, and agents from any and all claims, liabilities, actions, causes of action, costs, and expenses, including attorney's fees, and agree to indemnify and hold them harmless, arising out of or related to my participation in the program. This release includes, but is not limited to, any claims for personal injury, illness, or property damage.
3. Confidentiality: I agree to keep all tutoring sessions and related information confidential and not disclose any personally identifiable information about other participants.
4. Media Release: I grant the Hosts and their agents the right to use, reproduce, and/or distribute photographs, video, or audio recordings of me or my child participating in this learning activities for promotional purposes, without compensation or notification.
5. Agreement to Abide by Rules: I agree to abide by all rules and guidelines set forth by the Hosts in the learning activities. If a participant's behavior disrupts the hosts' instruction or the learning environment for other participants, after two warnings and notifications to parents without improvement, the participants may be dismissed from the program.
6. Governing Law: This waiver shall be governed by and construed in accordance with the laws of California. Any disputes arising from or in connection with this waiver shall be subject to the exclusive jurisdiction of the state and federal courts located in Los Angeles, California.

I have read this waiver and fully understand its terms and conditions. I acknowledge that I am signing this waiver voluntarily and with full knowledge of its legal consequences.
