Work Location: Various within London (usually clients' homes)

Essential Dutues and Responsibilities:
Deliver personalised day-to-day support, adhering fully to the client's care plan and working closely with the HCA Care Coordinator
Person-centred care: Commit to always deliver person-centred care
Respect: Total respect for client confidentiality and their right to person-centred care
Professional compliance: Always adhere to HCA policies and procedures, to ensure that care is delivered in compliance with all relevant laws and to ensure the well-being and safety of staff and clients
Personal Care: Assist with personal care, including skin care, bathing, shaving, oral care, toiletting (including management of bladder and bowels), dressing, hair care
Mobility: Enable mobility, including moving and handling transfers, pushing wheelchairs, and the checking of specialist equipment (e.g. wheelchair, hoist) as directed
Dietetics: Assist with menu planning, and the preparing, serving, and storing of foods
Daily living: Assist client in fulfillinf daily activities in the areas of work, leisure, education
Medication: Assist with giving medications, as prescribed by the client's medical team and as detailed in the client's care plan
Appointments: Dealing with medical appointments and correspondence as requested
Light domestic tasks: Day-to-day household tasks as deailed in client's care plan (e.g. changing beds, laundry, ironing, shopping and light dusting)
Recording: Record all activities carried out, as well as client’s mood and/or refusals/non-compliance and any visits from professionals, in the logbook at client’s home, as per HCA policies and procedures. Record all hours worked on the timesheet supplied by HCA, to be delivered to HCA weekly
Reporting: Keep in close communication with HCA regarding client care. Keep HCA updated with any concerns with or changes to the client’s condition or state, or any matters arising in your own life that may affect your work (e.g. time off for appointments). Report any safeguarding issues to HCA promptly.
Training: Commit to completing ongoing training. Actively assess areas of personal growth and set personal improvement goals with a Care Coordinator. Take initiative to seek learning opportunities to meet these goals.
