DEPOSIT

Please be prepared to pay a deposit when submitting this form. You will need to pay it *after* we select your appointment date together, you don't have to send it before that. Your deposit will be subtracted from the total amount when the tattoo is completed.

If you do not send me a deposit, then we do not have an appointment. I do not hold spots, so if you have to wait to send a deposit until pay day, I cannot guarantee that your date will remain open.

- Cash: bring by the shop
- I do not use other money transfer apps such as CashApp or Venmo, as we have had clients scam us before by disputing charges and taking their money back after tattoos have been completed.
*I am CASH ONLY at the shop, please be prepared. We have an ATM at the shop that charges a $3 fee on transactions up to $200. It charges a 3% fee on transactions between $200-$1000. If you'd like to avoid the fees, please stop at your own ATM beforehand.

CANCELLATION & RESCHEDULING POLICY

*All deposits are NON-REFUNDABLE. You may have ONE reschedule with the current deposit. Further reschedules will result in another $100 deposit being required to book. At that point, the first deposit is forfeit and will not be subtracted from the tattoo cost.

*In the event of a reschedule, please notify me at least 72 hours before your appointment. If you do not, your deposit will be forfeit & you will need to pay a new one.

*I do understand that emergencies happen, and I am willing to work with you in the case of a last minute emergency reschedule.

*Deposits remain on file for up to one year from the day it's sent. those 6 months.

*A no-call, no-show will lead to forfeiture of deposit and you will not be getting tattooed by me in the future -`♡´-

OTHER INFORMATION

*For the best tattoo experience, please make sure to eat before getting tattooed. Drink lots of water the morning of and the day BEFORE the tattoo. This ensures that your body is hydrated which means you will bleed less, your body will take the ink easily, and you will heal quicker.

*I have my own tattoo style. I put my own style into every piece I do. I will never directly copy someone's artwork or mimic the style of anyone else's work that is sent to me in references. If you want to book with me, then you want MY work.

*I am working at Black Altar Tattoo in Golden, CO. Directions to the shop.
