Please type without any symbols or spaces. Photo I.D. will be required on the appointment day to prove that you are 18. Failure to do so will mean you cannot get tattooed and will lose your deposit.

I understand that if I want to go ahead with a booking, I will be asked to pay a non-refundable deposit that will come off of the total price. (Reasons for a loss of deposit will be not showing up to the appointment, turning up late, cancelling within 72 hours, not having valid I.D., last minute drastic changes to the design and turning up in an unfit state to get tattooed)
I understand that a booking has not been confirmed until we have a date in the diary and a deposit to secure the date/design
