- Eligibility: This form is for NDIS clients who wish to attend AbilityFest. Please ensure you are registered with the NDIS and meet the event requirements.
- Age Restriction: Participants must be 16 years or older.
- Personal Details: Provide accurate personal information to assist us in organising transportation and accommodations if needed for statewide or interstate guests.
- Interests and Preferences: Indicate any specific interests or requirements you may have to help us tailor your experience.
- Transportation: If you need assistance with transportation to the event, please let us know. We will make arrangements based on your needs.
- Special Accommodations: Inform us of any special accommodations or support you may require during the event.
