Participant Information:

Acknowledgment and Assumption of Risk:

I, the undersigned, acknowledge that I am voluntarily participating in FRIEND YOUR FEAR and EQ LEADERSHIP breathwork and cold-water training sessions. These activities involve physical exercises such as deep breathing and ice-cold water immersion. I fully understand and accept the inherent risks associated with these activities, which include but are not limited to:

• Physical injury (e.g., muscle strains, sprains, or other bodily harm)
• Hypothermia or other temperature-related conditions
• Emotional or psychological effects
• Actions of other participants, volunteers, and facilitators
• Environmental conditions such as water temperature and weather
• Any unforeseen or unknown risks associated with breathwork and cold exposure

I understand and accept that these risks could result in serious injury or even death.

Certification of Fitness:

I certify that I am physically and mentally fit to participate in these activities and that I have not been advised otherwise by a qualified healthcare professional. I acknowledge that FRIEND YOUR FEAR and EQ LEADERSHIP do not claim any medical benefits or guarantees, and I participate at my own risk.

Health and Safety Responsibility:

I acknowledge that I am solely responsible for my own health and well-being during the event, including my physical, mental, and spiritual health. I confirm that I am in good physical condition and have consulted with a healthcare provider, if necessary, before participating in this event.

I agree to take 100% responsibility for monitoring my condition during the event and will stop participating if I experience any discomfort, pain, or adverse effects.

Waiver and Release:

In consideration of my participation in this event, I, for myself, my heirs, executors, administrators, successors, and assigns, hereby:

(A) Waive, release, and discharge FRIEND YOUR FEAR and EQ LEADERSHIP, their facilitators, employees, volunteers, sponsors, and any representatives (collectively referred to as "the Releasees") from any and all liability for death, disability, personal injury, property damage, or any other loss or damage incurred by me, whether caused by negligence or otherwise.

(B) Indemnify and hold harmless the Releasees from any and all claims, demands, actions, or causes of action, including attorneys' fees and costs, arising out of or related to my participation in the event, including but not limited to any injury, illness, or other damage that may occur before, during, or after the event, and any travel-related incidents while en route to or from the event location.

Consent to Medical Treatment:

I consent to receive medical treatment deemed necessary in case of injury, accident, or illness during the event. I understand that FRIEND YOUR FEAR and EQ LEADERSHIP do not assume responsibility for any costs associated with such treatment.

Photography and Media Release:

I consent to the use of any photographs, video recordings, or audio recordings taken during the event for promotional, educational, or other lawful purposes by the event organizers. I understand that I will not receive any compensation for the use of such media

Comprehensive Release:

This waiver and release shall be construed broadly to maximize its enforceability under applicable law.

Acknowledgment of Understanding:

I have read this waiver of liability, assumption of risk, and indemnity agreement. I fully understand its terms and acknowledge that I am signing this agreement freely and voluntarily. By my signature, I intend to be a complete and unconditional release of all liability to the greatest extent allowed by law.
