Mini-Tigerette Clinic Enrollment Form
We want to first say thank you for wanting to be a part of our Mini-Tigerettes Clinic this year! We are so excited and cannot wait to see everyone there! **Ages 4 years old through 8th grade**. Here are the details: Friday September 13th Registration: 5:30pm Dinner: 5:30-6pm. Clinic Time: 6:00pm-8:45pm. Show and Tell: 8:45-9:00pm *Show and tell will be held in the main gym and will be a chance to show family what they worked on all night! *Clinic cost is $40. This includes dinner (Hot Dog, Chips, water & cookie) and their clinic t-shirt that they get to keep! Shirts won't be handed out until game day on September 20th! The clinic will be held at Lebanon High School 510 Tiger Way, Lebanon, IN 46052. We will have check-in and have everyone enter through the gym doors of the high school. All you have to do is check-in and you are all set. This is an opportunity for your child to learn, dance and experience what it is like be a Tigerette! They will learn a dance to the school fight song and then learn a themed dance to "Happy"! They will get the opportunity to showcase all their hard work on SEPTEMBER 20th (which is also homecoming!) at a VARSITY FOOTBALL GAME for the pregame show. More details will be given out at check-in. See you there!

Tigerettes is where sport meets art. There is inherent danger in participation in any sport, including dance. All rules advised by the NFHS will be followed at all times to create a safe environment for learning and practicing skills. Participants will be instructed in safety methods prior to participating and will be supervised by clinic counselors at all times. Adults will also be present at all times to ensure your child’s safety and to take action in the event of injury or other emergency. My child(ren), may participate in the LHS Tigerette clinic held at LHS. I understand that LCSC, LHS, and all clinic counselors may not be held responsible for any injuries or detriment that my child incurs while participating in the clinic. I also understand that in the event of an emergency I will be contacted immediately to be informed of the situation and to provide direction for the care of my child. If I am not available, my emergency contact will be responsible. If no one of my choosing can be reached, I agree to allow LCSC staff to determine the best care for my child, including administration of first aid and discretion to take my child to the hospital if the need arises.
