Hello! Welcome to Z Fit Fitness. My name is Zavian Burns and I am elated that you have chosen me to be your personal trainer. Taking care of your body is not a quick fix, but a lifestyle of healthy choices. This program consists of all levels of fitness to maximize a complete workout. You are responsible for knowing your body. Please consult with your physician before starting my program. You are required to take before, progression and after pictures to document your journey. This is nonnegotiable. Payment is due prior to or in full before our training session begin! No refunds will begiven! I look forward to working with you. Thank you for this opportunity!

I release Z Fit Fitness LLC, Zavian S.J. Burns, its employees or volunteers from any demands, damages, actions or causes of action, either at law or equity of every nature and kind. I understand Z Fit Fitness LLC is not equipped to handle any medical situations or emergencies. Any sickness I encounter or any injuries I sustain, I will be responsible for. I have the approval of my physician to exercise. I agree to participate at my own risk with no liability to Z Fit Fitness LLC, Zavian Burns, its employees or any volunteers or the facility being utilized. I understand and give my permission to Z Fit Fitness LLC, Zavian S.J. Burns, it’s employees and volunteers the right to photograph before and after pictures, video and photograph our exercise routines for documentation and publication to build her profile as a certified personal trainer. I understand the terms of the agreement and I verify the above information as being true and correct.
