Order Form

- One date per order form. Please submit a new form for multiple date/s.
- Please ensure your selected item is check-marked and shows on the total amount.
- For special requests, assorted, bulk orders or different packaging in mind, please checkmark "Others" and input your notes and reference photos, if applicable. We will respond with the quote to you via email and/or your preferred contact channel.

Order Reminders:

• Please double-check your order if the selected product/s are accurate and are showing in the price.
• Please submit one form per date. 
• After submitting this form, we'll check your order and we'll revert thru email within 24-48 hours for confirmation of the product and payment. 
• Orders only become final once we confirm receipt of payment.
• NO IN-HOUSE DELIVERY. Products are available for pick-up, whether personal or through your booked courier (e.g. Lalamove, Grab delivery, etc.). Pick-up location is at One Gateway Place, Pioneer Street, Mandaluyong City. 

Product Reminders:

• Our products are freshly baked and do not have additional preservatives. It is best to be consumed immediately (1-3 days, refrigerated).  Especially, Ube Bars must be consumed or refrigerated immediately. 
• ALLERGY WARNING: Our products contain dairy, gluten and nuts.

Postponement/Cancellation/Refund Policy:

• Please coordinate directly and promptly with our team through Divine Treasures PH Facebook Messenger account as we would be happy to resolve your concerns and prevent possible food waste.
• As a small home business with limited capacity to store and sell, and with our items easily perishable, we hope you understand our policies as follows:
  • For Orders amounting to LESS THAN PhP2,000.00
    -NO refund/cancellation if notice is less than 24 hours from the pick-up schedule indicated in the order form.
    -We can approve full refund if notice is made more than 24 hours.
  • For Orders amounting to PhP2,000.00 AND UP
    -NO refund/cancellation if notice is less than 3 days from the pick-up schedule indicated in the order form.
    -We can approve full to 50% refund if notice is more than 3 days depending on the work and perishable supply stock already completed for your order, as applicable.
• For postponement of pick-up schedule where orders are already baked, we will only allow 24-hour delay in collecting your order, otherwise, we will have to give your order to the next taker.  

By completing this form, you agree that any personal information that you shared shall be accessed and used by the Divine Treasures Team for the completion of your order, and order data may be used to assess our own products.
