Credit Card: Payments are processed through PayPal. You may pay using an existing PayPal account, or any major credit card / debit card.
Checks: To pay by check, please make checks payable to Shoulder 360, Inc and mail to 5320 Ivafern Ln, La Canada, CA 91011. Checks must be recieved by March 21, 2025.
Cancellations: All conference cancellations must be made in writing by emailing to shoulder360course@gmail.com. Cancellations made by Monday, February 17, 2025 will receive a full refund minus a $30 processing fee. A 50% refund will be received if made between February 18 - March 2, 2025. Cancellations made after March 3, 2025 will not receive a refund.
Registration Rates:
- Physicians:
- $899 - Before December 13
- $999 - Between December 14 - January 31
- $1,250 - After January 31
- International Physician Attendees: $750
- Early Career Physician (less than 2 years out of residency/fellowship): $650
- Resident/Fellow/Medical Student: $400
- Advanced Practice Provider (PA, NP, APRN, PT, etc): $650
- Guest/Spouse of Conference Attendee: $200 (includes access to conference breakfasts, lunches & receptions)
- Faculty Dinner Ticket: $150 (Friday, April 4 @ Nikki Beach - ticket includes buffet dinner, open bar & entertainment)
