I understand, have read and completed this questionnaire truthfully. I agree that this constitutes full disclosure, and that it supersedes any previous verbal or written disclosures. I understand that withholding information or providing misinformation may result in contraindications and/or irritation to the skin from treatments received. The treatments I receive here are voluntary and I release this institution and/or the technician/esthetician/skin care professional from liability and assume full responsibility thereof.

Company Policies

 
We have a company policy of same day cancellation, same day rescheduling or no show fee of $30.00 for appointments less than 60 minutes. For appointments, laser or otherwise, 60 min or more you will be charged $60 - for that reason we do require a VALID credit card to be on your file when booking your appointment.**  Please give 24 hr notice to avoid charges.

 

If you RECEIVE A TEXT MESSAGE 2 DAYS PRIOR TO YOUR APPOINTMENT - WE ASK THAT YOU CONFIRM THE APPOINTMENT SO WE HAVE PROPER STAFFING - All appointments should be in your calendar - please do not rely on our email/sms reminders as a way of knowing when you have an appointment in case there are any glitches with the system.

 

WE DO NOT REFUND UNUSED SERVICES

 

For laser hair removal packages, once areas are treated, we will not be able to change the treatment areas.

 

THERE IS A 5 MINUTE GRACE PERIOD. Failure to show up on time may cause you to not be treated.

 

Please limit guests to 1 as we have limited seating in the waiting area and mindful of social distancing especially during COVID surges. 

Guests are not allowed in laser room. 

Children cannot be left unattended.

 

Gratuity for our technicians is not included but greatly appreciated - There is a “Gratuity” Box located at the front desk - the aestheticians accept cash/venmo/zelle
