Waiver/Release Agreement

We, Church/Team Name* , hereby agree to participate in the Dominican Republic Missions Trip* organized by Mission Zoom* , and we hereby acknowledge that there are certain risks and dangers associated with this activity. We acknowledge that these risks include, but are not limited to, injury, illness, and death, as well as loss or damage to personal property. In consideration of being permitted to participate in this activity, we agree to assume full responsibility for any risks, injuries, or damages that we may sustain, or that our property may sustain, as a result of our participation in this activity. We agree to release, waive, discharge, and hold harmless Mission Zoom* , its officers, directors, employees, and agents, from any and all claims, damages, or liabilities that may arise out of or in connection with our participation in this activity. We further agree to indemnify and hold harmless Mission Zoom* , its officers, directors, employees, and agents, from any and all claims, damages, or liabilities that may arise out of or in connection with our participation in this activity, including but not limited to, claims made by third parties. We understand that this Liability Waiver and Release Agreement is a binding legal agreement, and we sign it voluntarily and with full knowledge of its contents. We acknowledge that we have read and fully understand the terms and conditions set forth in this agreement.

We grant Mission Zoom the right and permission to use photographs and/or video recordings of individuals and our team on Mission Zoom and other websites and in publications, and promotional flyers.

We understand and agree that such photographs and/or video recordings may be placed on the Internet. We waive the right to approve the final product. We agree that all such photographs, video and audio recordings, and any reproductions will remain the property of Mission Zoom.
