WHO: STUDENTS MUST MEET THE FOLLOWING CRITERIA IN ORDER TO ATTEND HOMECOMING:

*Attendees must be between the ages of 14-19 years old as of Sept 1, 2024 AND at least a freshman in high school (8th graders taking 9th grade courses will qualify for this event) for the 2024-2025 traditional school year.

TICKET RESELL POLICY:

50 Shades of Slay Productions host sellout events.

We do not allow ticket transfers on HOMECOMING TICKETS. 

We will NOT HONOR ANY TICKET TRANSFERS. 

All ticket sales are final. 

The dress code will be STRICTLY enforced. By completing this registration form, you agree to adhere to all dress code policies. Any violations will result in forfeiting your ticket and/or removal from dance.

 
Gentlemen: Dress Pants or Non-ripped dark denim pants, collared shirts, suits, etc.

 
Ladies: Dresses must be in good taste-fitted dresses must be no shorter than mid thigh. (Ladies are not required to wear dresses.)

• Must be 6 INCHES from INNER THIGH or longer, to include length of slits, no non-covered midriff, no see through fabric, modest coverage of cleavage. 
• “Dress necklines must be cut in a modest way without showing excessive cleavage” and any cuts in the back or sides “must not be cut below the navel (below your elbow).”
• Undershorts/biker shorts are highly encouraged.

*** Students may not change into casual attire during the dance. Doing so is a direct violation of our dress code policy. ***

Leaders/volunteers at this event may ask an attendee of the dance to leave if attire, behavior, and/or speech are considered inappropriate based on our judgment. No refunds will be issued.

No provocative dancing. No outside food/beverages. No alcohol, tobacco products, or smoking/vaping. No public display of affection. No changing of attire. This is a formal event for high school students.

This applies to behavior inside and outside of the venue.

 
Parents/Guardians are responsible for any damage caused by their student to private or public properties.

• Drop off time is 6 PM, 6400 Maple Avenue, Suite 850, Dallas, TX 75235.
• Attendees must depart by 8 PM.
• Students who chose the option that included dinner & shuttle will be shuttled from the venue at 8 PM to the restaurant.
• Restaurant pickup is on or before 10 PM. 

• Acknowledgment of Risks: I acknowledge that the Homecoming celebration hosted by 50 Shades of Slay Productions ("the Event") is an event that involves inherent risks, including but not limited to the risk of transmission of Covid-19 and its variants. By allowing my student to participate in the Event, I voluntarily assume all risks associated with participation, including but not limited to illness, injury, or accident.

• Release of Liability: I understand that this Accident Waiver and Release of Liability form will be used by the event holders, sponsors, and organizers, and it will govern my student's actions and responsibilities during the Event, including travel to and from the Event.

• On behalf of my student(s), myself, our heirs, executors, administrators, and assigns, I hereby waive, release, and discharge 50 Shades of Slay Productions, its directors, officers, volunteers, representatives, and agents from any and all liability, including but not limited to claims, damages, demands, actions, or causes of action of any kind, arising from or related to participation in the Event.

• Indemnification: I further agree to indemnify and hold harmless 50 Shades of Slay Productions, its directors, officers, volunteers, representatives, and agents from any claims or liabilities arising out of or resulting from my student's participation in the Event.

• Medical Treatment Consent: In the event of injury, accident, or illness during the Event, I hereby consent to my student receiving medical treatment deemed necessary by qualified medical personnel.

• Image/Photo Release: I understand that my student may be photographed, videotaped, or otherwise recorded during the Event. I agree that 50 Shades of Slay Productions may use these images or recordings for any legitimate purpose, including promotional materials, without compensation or further notice.

• Broad Interpretation and Venue Affiliation: This Accident Waiver and Release of Liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law. I understand that the Event venue is not affiliated with 50 Shades of Slay Productions and assumes no responsibility for the Event.

• Acknowledgment and Agreement: By signing below, I acknowledge that I have read and fully understand this waiver. I am aware that I am giving up legal rights by signing this document and agree to its terms freely and voluntarily. Please type and sign your full name below to reflect your understanding and agreement with our above waiver.

This form does not guarantee a ticket to the dance. PAYMENT must be made upon completion of the form. A form without the following payment will be voided after 24 hours.  Payment must reflect ticket price.
