1. How would you describe your current level of self-awareness and inner work?*

I am fully committed to personal growth and have engaged in deep healing practices for past wounds and limiting beliefs.I’ve done some inner work, but there are still areas where I feel challenged or uncertain.I’m just starting to explore inner work and am seeking guidance on how to proceed.I don’t often prioritize self-awareness or inner work.
2. When faced with challenges or uncertainty, how do you typically respond?*

I remain centered and grounded, trusting in my inner wisdom and the unfolding of my path.I experience some fear and self-doubt, but I am learning to navigate these feelings with greater resilience.I often feel overwhelmed and reactive, struggling to maintain a sense of peace and perspective.I frequently feel controlled by my emotions and have difficulty managing my reactions.
3. How aligned do you feel with your soul's purpose and the impact you want to make?*

I have a clear vision for my purpose-driven work and am actively pursuing it.I sense a higher calling but am still exploring what that looks like in my life and career.I feel disconnected from my purpose and often question the meaning of my work and life.I haven’t yet considered my life’s purpose or the impact I want to create.
4. How would you describe your relationship with your intuition and inner guidance?*

I have a strong connection to my intuition and use it regularly to guide my decisions and actions.I am learning to trust my intuition, though I sometimes confuse it with fear or ego-based thinking.I rarely consider my intuition and often rely on external expectations for decision-making.I feel disconnected from my intuition entirely.
5. How effectively do you set and maintain healthy boundaries?*

Excellent - I am clear about my boundaries and communicate them confidently.Good - I have established some boundaries but could be more consistent in enforcing them.Fair - I struggle with setting boundaries and often find myself overextended.Poor - I have very few boundaries and frequently feel taken advantage of by others.
6. How much inner work have you done to heal past wounds/traumas and release limiting beliefs?*

Extensive - I have actively worked on healing past experiences and reprogramming limiting beliefs.Moderate - I’ve addressed some issues but know there’s more work to be done.Minimal - I have only briefly explored my past wounds and limiting beliefs.None - I haven’t yet tackled my past wounds or reprogrammed limiting beliefs.
7. How aligned are you with your core values and authentic self?*

Completely aligned - I live in full integrity with my values and true self.Mostly aligned - I generally align with my values but sometimes compromise them.Somewhat aligned - I am aware of my values but often struggle to live by them.Not aligned - I feel disconnected from my values and authentic self.
8. How committed are you to personal growth and lifelong learning?*

Extremely committed - I consistently invest in practices and resources to expand my knowledge and awareness.Somewhat committed - I engage in personal growth when possible but it’s not always a priority.Not very committed - Personal growth is not a current focus in my life.Not at all committed - I resist personal growth and prefer to stay in my comfort zone.
9. How willing are you to step outside your comfort zone and embrace change and the unknown?*

Very willing - I see change as a growth opportunity and am ready to take bold steps.Somewhat willing - I am open to change but feel hesitant about leaving the familiar.Not very willing - I resist change and prefer to stick to established routines.Unwilling - I avoid anything that disrupts my comfort zone or sense of control.
10. When starting your transformational journey to 5D Leadership, how do you prefer to travel, and what are you willing to invest in your growth?*

First Class: I seek an immersive experience with retreats and workshops and am ready to invest significantly in my transformation.Business Class: I value personalized coaching and am prepared to invest in one-on-one support.Premium Economy: I prefer a blend of self-paced learning and structured support, such as digital courses and group coaching, with a moderate investment.Economy: I've been relying on free or low-cost resources, but I'm beginning to recognize that transformative growth might require a different approach. I'm open to re-evaluating my investment in myself and exploring options that could accelerate my journey to 5D Leadership..
