I hereby consent myself/my child SAWS (Swimming and Water Safety) program at Bay of Islands Recreation Centre . I understand that Bay of Islands Recreation Centre Management and its officers, staff and agents shall be released from, and shall not incur any responsibility or liability whatsoever for any accident of injury to the participant or for any damage to or loss of property of the participant. I hereby authorise Bay of Island Recreation Centre staff to organise the enrolment at my expense. I have read and understood the Terms and Conditions on this form.

SAWS (Swimming and Water Safety) Terms and Conditons
To ensure that every participant receives the maximum benefit from the programme and services at the centre, please take time to read through the following SAWS (Swimming and Water Safety) Terms & Conditions, relevant to the Cbec and Belgravia Leisure SAWS (Swimming and Water Safety) Programme.

Please remember children under 10 years must be accompanied by an adult and be placed in their classes. Please ensure children have gone to the toilet prior to lessons. Children should not enter the water before commencement of their class and should depart the water at the completion of their lesson.

Payments need to be made on enrolment to confirm your space in the class. This can be done in full or on our Direct Debit (with correct bank details), where debits will occur on a weekly basis.

Direct Debit Option $10.00 per lesson for SCHOOL AGE

- First week's payment required upon booking -Direct Debit form to be submitted by first lesson at reception. After the third consecutive rejection (facility) reserves the right to cancel any direct debit and associated classes. - If you would like to cancel your lessons you need to fill out cancelation form. There is a 7 day notice period for any cancellation. - DD Lessons roll over to the next term- if you don't want to continue in the next term please fill out cancelation form.

Full Payment Fees: https://www.bayofislandsrecreationcentre.co.nz/ $100 for 10 lessons- SCHOOL AGE

-Fees are paid in full at booking time. (No cancelations once term commences)

You are entitled to have 1 make up lesson subject to availability, and you may require a medical certificate. If you are booked into a make-up and do not show the makeup lesson is automatically forfeited.
Cbec and Belgravia Leisure reserves the right to replace instructors, change/cancel classes should the need arise. This may occur due to class numbers, illness of teachers or other extending circumstances and will be dealt with accordingly. Cbec and Belgravia Leisure further reserves the right with absolute discretion to refuse admission to any person/or direct any person to vacate the centre, and Bay of Islands Recreation centre will not be liable in anyway.

By signing above, you agree to the Terms and Conditions of Bay of Islands Recreation Centre, as outlined above as amended by the centre at their discretion.
