What is Butterfly Lash Membership? Membership gives you access to discounts and establish you as a loyalty customer. We honor and value ALL OF OUR CUSTOMERS especially our long term committed customers. Butterfly Lash membership includes 1x 3-4wk refill $74 or 2x monthly refills at only $99. Customers have the choice to select either a classic set, hybrid, or a volume set.

Benefits Loyalty reward points, discounted prices, free featured products, free removal services, points earned for referrals

Clients enroll in membership benefits on initial set, registration forms and ACH/credit card payment agreement must be signed. Refill payment will draft from member's account on a monthly basis. Customers are responsible for scheduling monthly refill appointment.

By signing this agreement, I am confirming my enrollment in the Butterfly lash Membership at Butterfly Lash Lounge.

I understand my account will be drafted $74 for platinum members or $99 for Elite members for my refills on the last day or 1st Through 5th of every month.

My initial set will be paid upfront at a regular cost.

I understand that I am responsible for scheduling my refills every month or obtain a refill no later than 25days past my last lash extension Fill.

I have signed an addition ACH agreement and understand these terms and conditions.
