The undersigned hereby requests to be supplied with water and/or sewer services by the Pablo Water & Sewer District (the District) at the address stated above. I agree to comply with all of the rules and regulations of the District. I AGREE TO CLAIM NO DAMAGE ON ACCOUNT

OF THE STOPPAGE OF THE FLOW OF WATER IF CAUSED BY ACCIDENT OR IF

NECESSARY TO MAKE ALTERATIONS, REPAIRS, OR IMPROVEMENTS. I agree to keep all plumbing and fixtures on my premises in repair and promptly stop all leaks.

Any leaks from the curb stop to the property owners home shall be the responsibility of the property owner. For any ongoing leaks the property owner will be notified three (3) times; first notification will be on their bill, second notification will be with a physical visit by a representative of the District, and third notification will be by certified letter. After the three (3) notifications, and if the leak continues for a period of 2 months, or longer, the property owner will be billed a fee of $500.00 (leak fee) per month until the leak has been fixed. In addition to the $500.00 leak fee the property owner will still be liable for the monthly water and sewer billing.

If for any reason you are temporarily disconnected by the District, you will still be responsible for payment of the basic monthly water and sewer fees.

All renters and delinquent landowners will be required to make a deposit equal to 2 months basic water and/or sewer charges. Deposits are as follows: Water & Sewer - $200.00, Sewer Only - $100.00, Water Only - $100.00.

I agree to pay for water and or sewer services used at the 3/4" service rates as follows:

$38.06 FOR 5,000 GALLONS AND $1.31 PER 1,000 GALLONS AFTER 5,000

$21.21 BASE AND $3.25 PER 1,000 GALLONS OF WATER USED

SUMMER IRRIGATION ALLOWANCES IS THE WINTERTIME 6-MONTH

AVERAGE. (Wintertime usage months are October through March) The District establishes the charges. I agree to pay my water and sewer bill by the 26th of every month. I also understand that if a bill is one day past due there will be a $10.00 late charge applied to the bill.

I agree to pay all late fees, reconnect charges, and all litigation costs incurred by the District if legal means are required to collect on an outstanding bill. I agree to allow any representative of the District to read, inspect, and service the meter on the premises on any day between the hours of 8:00 AM and 4:30 PM, except on Saturdays and Sundays, unless there is an emergency. If the District considers it necessary to ration or restrict the use of water during any hours or period of time, I agree to comply with any and all such rationing and restrictions.

For failure to comply with this agreement, or any part thereof, the District may, in addition to other remedies, may elect to pursue, and/or shut-off the supply of water to the premises after ten (10) days with written notice sent to me by regular mail at the address stated above. If it becomes necessary to disconnect my services, there will be a $25.00 disconnect and $25.00 reconnect charge added to my bill. The District has adopted a fifteen-dollar ($15.00) fee for all insufficient fund checks (NSF) returned. Two NSF checks will result in cash, money order, credit card (along with small fee for handling of credit card payments), or cashier's check being the only acceptable form of payment method.
