If no request is submitted, awards will be not be provided. All youth leagues will receive end of season awards.
If you would like end of league ending awards, please submit this form by NOVEMBER 1 to our office. Once on file, we will continue each year. If you wish to change your options, please submit a new form. Thank you!
End of season award packet includes:
- League Champion award – one per team member
- High Series award*– 2 (male/female)
- High Game award* – 2 (male/female)
- High Average award* – 2 (male/female)
- Most Improved award* – 2 (male/female)
*We will adjust the quantity based on the league format.
