Assumption of Risk: I understand that participating in a baptism involves physical activities that carry inherent risks, including but not limited to, slipping, falling, or other accidents that could result in injury. I assume full responsibility for any risk of bodily injury, death, or property damage arising out of or related to this event.
Medical Treatment: I certify that I am physically fit, have sufficiently prepared or trained for participation in this event, and have not been advised to refrain from participation by a qualified medical professional. I consent to receive medical treatment that may be deemed advisable in the event of injury, accident, and/or illness during this event.
Release and Waiver: In consideration of being permitted to participate in the Community Water Baptism, I hereby release, waive, discharge, and covenant not to sue Norchell Evans Ministries, its leaders, directors, officers, employees, volunteers, agents, and assigns, from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury that may be sustained by me, whether caused by negligence or otherwise, while participating in this event or in any activity related to this event.
Photographic Release: I hereby grant Norchell Evans Ministries permission to use my likeness in photographs, video recordings, or any other media taken during the event for promotional purposes, without compensation.
Acknowledgment of Understanding: I have read and understand this liability waiver and release, and I fully agree to its terms. I understand that by signing this waiver, I am giving up substantial rights, including the right to sue. I acknowledge that I am signing this agreement freely and voluntarily, and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
