As a participant in the certificate program, you are eligible to receive one free course due to being part of a group of five individuals attending the same church. This benefit is granted under the condition that:

• Eligibility: This offer is available only to individuals enrolled in a degree program at ACTS.
• Group Requirement: The free course benefit applies to the first course taken as part of the certificate program, provided that all members of the group of five are enrolled in the same certificate program.
• Non-Transferability: This free course offer is non-transferable and can only be redeemed by the individual who signed this form.
• Group Discount is applicable for one year only, and evaluated annually.
• If one or more individuals drop or withdraw courses after the first semester, the group discount may no longer apply.
• Students must remain in good academic standing to continue to receive discounts (not on academic probation, etc.)
