PURPOSE
This waiver grants permission for the use of the child’s name, likeness, image, voice,
and/or appearance in social media posts and other digital content related to Atlanta Vipers.
CONSENT
I, the undersigned, hereby grant permission to Atlanta Vipers to capture, record, and
use my child's image, likeness, voice, and/or appearance in any photographs, video recordings, digital images, and other media formats taken during Atlanta Vipers activities and events.
USAGE
I understand that these recordings and images may be used in social media posts,
promotional materials, website content, newsletters, and other forms of public communication.
RIGHTS
I acknowledge that Atlanta Vipers holds the right to use, reproduce, edit, and distribute these images and recordings without limitation. I also understand that neither I nor my child will receive compensation for such use.
PRIVACY
I am aware that once my child’s image or information is posted on social media,
Atlanta Vipers cannot control the distribution and use of the content by others.
Release of Liability: I release Atlanta Vipers and its representatives from any claims or liabilities related to the use of my child's image or recordings, including but not limited to claims for libel, invasion of privacy, or misappropriation.
OPT-OUT
I understand that I have the right to withdraw my consent at any time by providing
written notice to Atlanta Vipers. However, I acknowledge that withdrawal of consent will not apply to materials already in use.
ACKNOWLEDGMENT
By signing below, I certify that I have read and fully understand this waiver
and consent form, and that I agree to its terms.
