When you submit this form, each studio will be notified. If you do not have a current account with each studio please go online and create those accounts using the same information you used on this form. Once that is completed - each studio can activate your account and charge your card.

You are signing up for an unlimited class membership to 7 Minutes Fitness, Cycle Haus Nashville and Hot Yoga East Nashville. This is a 3 month minimum comittment and is non refundable. Membership auto-renews monthly until member cancels in writting with each individual studio. Cacenellations needs to be made 72 business hours before the next payment is drafted from the account.

You agree to the late cancel and no shows fee per each studios individual policies (please see the websites for these studios for more information).

You understand that to book classes you will need to book directly through each studios websites or branded apps.

You understand this form will be distributed to each studio who will charge their own monthly fee of $88 or $95 for the above chosen membership.

You agree that cancelling this membership cancels your membership with all studios and that this membership authorizes you to use all studios at an unlimited class rate per month.
