PART A – DCAHL
Each person ("The Player") assumes all risk of personal injury that may result from participation in all activities of the Durham Central Adult Hockey League. The Player will not hold the Durham Central Hockey League, Tribute Communities Centre, City of Oshawa, or OVG360 or any of their officials, staff, or owners liable for injury that the Player may sustain while participating in any DCAHL activity. The Player understands and agrees that the sport of ice hockey has physical dangers that may result in serious injury or death. The Player is advised to carry personal medical insurance. The Player certifies that he/she has no known medical condition that would prohibit him/her from playing the sport of ice hockey. The Player agrees to wear specified equipment set out in the DCAHL Rule Book. The Player agrees that he/she will act in a responsible manner in all DCAHL activities. The Player agrees to reimburse the DCAHL, in full within five days of notice, for the cost of any property damage for which the Player is held responsible by the DCAHL staff or Tribute Communities Centre Management. The determination of such responsibility shall be binding on the Player.
The DCAHL Management has the final authority as to what divisions and times the Teams and Players will participate in. The Player understands and agrees that he/she may be expelled from the DCAHL for any of the following reasons: 1) Financial delinquency; 2) Failure to abide by DCAHL rules; 3) Falsification of registration information. The Player agrees to the DCAHL policy that there is no reimbursement to any Players, regardless of reason for dismissal.
PART B – CITY OF OSHAWA
Each person using the ice facilities of City of Oshawa assumes all risk of personal injury that may result from participation in any activity conducted in any City of Oshawa facility. The Player will not hold the City of Oshawa, or any of its officials, staff, owners or the proprietor or employees of any City of Oshawa facility liable for injury that the Player may sustain while participating in any of the aforesaid activities. The Player understands and agrees that any sport or other activity conducted on ice has physical dangers that may result in serious injury or death. The Player is advised to carry both primary and extended medical insurance. The Player certifies that he/she has no known medical condition that would prohibit him/her from engaging in any activity on ice.
PART C – SENTINEL RISK INSURANCE GROUP
Effective 2017
In conjunction with the DCAHL, Sentinel Risk Insurance Group notes that each person using the ice facilities of City of Oshawa will receive Accidental Death and Dismemberment coverage as part of the Durham Central Adult Hockey League at no additional cost to the individual. To be eligible for insurance coverage, each Player must complete this waiver form and have a copy of a piece of photo identification on hand at Tribute Communities Centre. The information provided in this process may be used in registering each Player with the insurance provider. Failure to complete this process in its entirety or providing any false or misleading information or omitting any material information will nullify the participant from insurance coverage with the DCAHL and its insurance carrier or may result in denial of coverage by the insurer.
All DCAHL Players are encouraged to request and review the current Accident Coverage Policy brochure, available through the DCAHL Manager.
PART D – PAYMENT TERMS AND CONDITIONS
ALL TEAM ENTRIES:
A minimum $1,500.00 deposit must accompany your registration to be guaranteed a spot. The balance will be payable according to the payment terms of the DCAHL. It is the responsibility of each Player in the DCAHL to learn these payment terms by speaking with their Team Rep. All late payment amounts will be provided to the associated Team Rep and his/her team will not be able to play their next game until that outstanding amount is cleared. As of the FINAL PAYMENT DATE, the Durham Central Adult Hockey League reserves the right to charge all outstanding amounts to the credit card provided by the registrant. Post-dated cheques are not exempt from late fees. No refunds on initial deposits.
PART E – PRIVACY POLICY
Personal Information Protection: Tribute Communities Centre collects and uses personal information in line with Province of Ontario's Privacy Principles. By providing us with your information, you consent to Tribute Communities Centre’s use of the information in providing you with products or services and/or information about products or services. Your information will NOT be shared with any other organizations or persons not affiliated with Tribute Communities Centre. All persons will have the ability to opt out of any marketing material that they may receive.
PART F – ACCEPTANCE OF DCAHL TERMS AND CONDITIONS
I have read the above terms and conditions thoroughly and understand them. No oral representations or statements or inducements have been made to me that change, alter, or modify anything within the written agreement.
