I, the cardholder, hereby authorize Elite Distributors to charge my credit card and store my payment information in their system for current and future orders on an ongoing basis. I confirm that the credit card details and billing address provided are complete and accurate. I understand that any issues or disputes must be addressed directly with Elite Distributors, and I agree not to initiate any chargebacks.

I have been informed that I may cancel my account and request the removal of my payment information from Elite Distributors' database by phone or by signing a consent form provided by them.

I also acknowledge that I am personally responsible for any unpaid or past-due invoices, including those subject to chargebacks, even in the event of the company’s non-payment or insolvency. I agree to act as a personal guarantor for any outstanding balances owed to Elite Distributors.
