Responses submitted through this form may be shared on our website, social media, grant reports, fundraising campaigns, and miscellaneous marketing materials. Hearing directly from clients helps us share about the work that we do and further our mission to provide affordable psychedelic-assisted therapy to underserved communities, as well as high-quality training to diverse therapists.
We prioritize client confidentiality and the boundaries of the therapeutic relationship, so please do not include your name or the name of your specific Alchemy therapist(s). All responses are optional and anonymous, and participants will receive no compensation or other incentive for their responses. Because this is an anonymous form, once a response has been submitted, you will not be able to delete your entry.
