my permission to participate in the Museum of Indian Culture’s Native American Heritage Day. I hereby waive and release the Museum of Indian Culture and all individuals, staff members or volunteers working in connection with Native American Heritage Day activities from any and all possible claims for injury to person or property which might arise in connections with my child’s participation in activities sponsored or provided by the Museum of Indian Culture. I do not hold the Museum of Indian Culture responsible for any accident or illness which might occur and agree, if necessary, that it is my responsibility as parent / guardian to secure the service of a doctor at my expense. I understand that my child must be accompanied by a parent or guardian at all times. Note: This is an all-day outdoor event. Don’t forget to wear sunscreen and please dress your child accordingly.
