• POLICIES AND PROCEDURES AGREEMENT

    Please read and sign this agreement below.
  • Driver must be 15 minutes early to job sites. Personal use of company equipment after hours is prohibited. If driver uses company truck for personal use during and after business hours, driver is subject to termination and or a $1500 fine and or up to $5 hourly pay decrease for up to 2months.

    Driver is prohibited from operating company equipment while intoxicated. If Driver is under influence of drugs alcohol and firearms is subject to Termination. Driver must perform safety Pre and Post Trip inspection logs. Driver must submit all Pre and Post Trip inspections log daily.

    Failure to submit Pre and Post Trips will result in a pink slip. All Drivers are given 3 pink slips. The third pink slip will result in suspension or termination.

    No call no show will result in automatic termination. Driver must give 48 hour written and or verbal notice to take off from job. Driver must give written 2 weeks notice of resignation and failure to do so will result in 50% deduction of last payment.

    Driver is prohibited from wearing shorts, sandals, and tank tops. Driver must have pants, shirt, steel toe boots, hard hat, safety vest and gloves. Driver must keep tools in truck for emergency and maintenance.

    Driver must read and sign policy handbook. Make up days can be any day of week. Including weekends are mandatory. 

    While we do understand the wear and tear each job can potentionally cause to the truck, we do ask drivers to handle the truck with care at all times. Drivers are responsible for any damage done to the truck that was not caused by the job. Addionally, If asphalt is spilled and company is charged the fee will come out of driver’s pay. Ride alongs are prohibited. In the event of an accident the unauthorized rider is not covered under insurance policy. MPHC Equipment Lost or stolen will be deducted from driver’s pay. If driver quits job he or she is required to take equipment back to the yard. Driver should always contact Manager or Broker should an altercation happen while on job.
