Arcadia Pet Resort Waiver of Liability and Consent

I, the undersigned, acknowledge and agree that:

Arcadia Pet Resort will exercise reasonable care for the health and safety of my pet but is not liable for any illness, injury, loss, or damage that may occur while my pet is in their care due to unforeseeable or uncontrollable circumstances.
My pet is in good health, free of communicable diseases, and up-to-date on all required vaccinations, including rabies, distemper, and parvovirus (DHPP). I will provide proof if requested.
My pet does not have a history of aggressive behavior. Arcadia Pet Resort reserves the right to refuse services due to behavioral, health, or vaccination concerns.
In the event of an emergency, I authorize Arcadia Pet Resort to seek veterinary care if I or my emergency contact cannot be reached, and I will be responsible for all related costs.
I will inform Arcadia Pet Resort of any special needs, behavioral issues, or medical conditions. I understand they may refuse care if my pet poses a danger.
I consent to Arcadia Pet Resort taking and using photographs or videos of my pet for promotional purposes.
By signing below, I confirm that I have read, understood, and agree to these terms, and this agreement remains in effect for all future visits unless revoked in writing.
