Thank you for informing us of your decision not to return! We hope you and your little one have enjoyed our ballet classes and that they (and you) have made some wonderful new friends. To proceed with the process of your departure from our programme, we kindly ask that you submit this form no later than the first Friday after the term concludes. This time frame allows us to fill your child’s position in the class, plan for the hiring of facilities and ensure that we have the necessary teachers and resources available. Please be aware that forms submitted after this deadline will result in fees for the following term being deducted from your account. If you are leaving us at the end of Term 4, a class cancellation request is not required, as your enrolment will not automatically roll over into the following year.
