Acknowledgment of Risk:
I, the undersigned, understand that participation in rhythmic gymnastics activities, training, and events involves physical exertion and carries inherent risks of injury, including but not limited to sprains, strains, fractures, concussions, or other serious injuries.
I acknowledge that I, or my child, am voluntarily participating with full knowledge of these risks, and I agree to assume all risks of personal injury, illness, or property damage that may occur during participation, whether on or off the premises, during practices, camps, events, performances, or travel related to the Grand Rhythmic Gymnastics Academy.
Consent to Physical Contact and Assistance:
I understand that rhythmic gymnastics training may require hands-on correction, stretching assistance, spotting, and physical guidance from the coach and staff. I hereby give permission to the coach and authorized staff to touch, stretch, and adjust myself or my child for the purpose of improving technique, ensuring proper form, and preventing injury.
I further understand that in the event of a minor injury (e.g., sprain, muscle cramp), the coach or staff may administer basic first aid, such as applying ice, bandages, or providing physical support.
Medical Information & Emergency Treatment:
I certify that I, or my child, am physically fit to participate in rhythmic gymnastics and have been cleared by a licensed physician if necessary.
I authorize Grand Rhythmic Gymnastics Academy to seek emergency medical treatment in the event of an accident or sudden illness. I release the Academy, its coaches, and staff from liability for any medical treatment rendered in good faith. I assume full responsibility for any medical expenses incurred as a result of participation.
Release of Liability for Unforeseen Incidents:
In consideration of my, or my child’s, participation in Grand Rhythmic Gymnastics Academy, I hereby release, discharge, indemnify, and hold harmless the Academy, its owners, coaches, staff, and representatives from any claims, demands, or causes of action arising out of participation, including but not limited to injuries, illnesses, or property damage.
This waiver applies to the fullest extent permitted by law and includes all activities occurring on the premises, at off-site events, and during supervised travel.
I acknowledge that unforeseen incidents may occur and voluntarily assume all risks associated with participation. I agree not to pursue legal action against the Academy, its coaches, staff, or representatives.
Media Release:
I hereby grant permission for the Grand Rhythmic Gymnastics Academy to use photographs, videos, or other media of myself or my child taken during club activities for promotional and marketing purposes, including social media, website, and printed materials.
Payment Policy:
I understand and agree to the following payment terms set by Grand Rhythmic Gymnastics Academy:
Tuition Payment Schedule
Tuition is due in advance of services and must be paid monthly on or before the 1st of each month. Payments received after the 5th of the month will incur a $25 late fee, unless prior arrangements have been made in writing.
Non-Refundable Tuition
All tuition payments are non-refundable, regardless of missed classes, holidays, or voluntary withdrawal. Refunds or credits will not be issued for absences or cancellations.
Make-Up Classes
Make-up classes are offered based on availability and are not guaranteed. Missed classes must be made up within 30 days and scheduled in advance. No credits or refunds will be issued for unused make-ups.
Registration and Fees
A non-refundable annual registration fee is required for all gymnasts upon enrollment and every 12 months thereafter. Other fees (such as uniforms, performances, or competitions) will be communicated in advance and must be paid by the stated deadlines.
Delinquent Accounts
If an account becomes more than 30 days overdue, the student may be temporarily suspended from participation until the balance is paid. Continued non-payment may result in removal from the program.
Payment Methods
We accept payments through [credit/debit card, Zelle, check, etc.]. All payments must be clearly marked with the gymnast’s name.
Withdrawal Notice
A 14-day written notice is required to withdraw from the program. If no notice is given, the account holder will be responsible for the following month’s tuition.
Right to Refuse Service and Terminate Enrollment
Grand Rhythmic Gymnastics Academy reserves the right to refuse service, deny participation, or terminate the enrollment of any student at any time, with or without cause, at the sole discretion of the Academy.
Reasons for termination may include, but are not limited to, disruptive behavior, failure to comply with Academy rules or staff instructions, safety concerns, or conduct deemed inappropriate or detrimental to the learning environment.
Any unused tuition or fees may be refunded at the discretion of the Academy.
Severability & Jurisdiction
If any provision of this Agreement is found invalid or unenforceable, the remaining provisions shall remain in full effect.
This Agreement shall be governed by the laws of the State of California, and any disputes shall be resolved within San Diego County, California.
Acknowledgment of Policies:
I acknowledge that I have received, read, and agree to abide by the Grand Rhythmic Gymnastics Academy rules, policies, and guidelines, including safety protocols, dress code, and attendance policies.
By my signature, I indicate that I have read and understand this Waiver of Liability and that by signing this Waiver of Liability, I am voluntarily surrendering certain legal rights. Thus, I am aware that this is a waiver and a release of liability and I voluntarily agree to its terms.
By signing below, I represent that I have legal capacity, that I am the parent and/or legal guardian of the below-named Gymnast, and that I am authorized to act on behalf of the Gymnast.
