Do you have general liability AND workers comp insurance. Note: In most cases, Paragon clients require contractors performing work on-site to have general liability AND workers compensation insurance (including self-employed sole-proprietors and single-member LLCs with one employee who are not legally required to have it).*

YesNoI have General Liability but I am an independent contractor (owner/operator with no employees that work on-site) and do not have Workers Compensation Insurance (note: this will limit Paragon's ability to recommend you to other clients).
Sole proprietors with no employees waive the right to file workers compensation claims against Paragon Management Group and its clients, and are personally responsible for any injuries/illnesses sustained while performing services to said entities. Paragon will not refer vendors who do not have Worker's Compensation Insurance for additional work in to protect our clients from liability.
How will we receive your certificate of insurance (COI):*

My insurance company will send my certificate(s) of Insurance to coi@paragonky.com (please have your agent cc you so that you are notified when sent).I will upload it below (multiple files can be uploaded if you have multiple certificates)
Please note a new certificate is required upon each renewal.

Please have your agent add COI@paragonky.com to the list of companies who need to have renewals automatically sent upon each renewal to remain an active vendor

FINANCIAL INFORMATION & AGREEMENT

IMPORTANT: Invoices must be addressed to and "billed to" the community/HOA's name. If you perform work for multiple communities, Separate billing profiles with names and account numbers must be set up for for each community. Invoices not billed correctly will not be paid. Paragon cannot make payment off of Statements.

ACH/direct deposit (highly recommended) I hereby authorize Paragon Management Group to process electronic deposits into the bank account indicated below. This authorization will remain in effect until I notify Paragon Management Group in writing to discontinue electronic deposit of funds. I hereby certificate that by signing below that I have the authority to authorize these electronic deposits.Check (strongly discouraged) IMPORTANT: Paragon cannot guarantee USPS will deliver checks. Payment terms may also not be achievable. Vendors must wait 30 days from issuance for a check to be delivered before requesting a replacement. Reissued checks are subject to a $35 stop payment/reissue fee. Vendors generally will not be able to pick up checks at our office. By selecting this option, you consent to these terms.
