Faith Formation Mass Attendance

This form is to be used when you visit another parish for Mass and you are unable to sign-in to St. William's using the QR Codes. You will also be asked to upload a photo of your kids with with the parish bulletin or in front of the Catholic church you visited. Submissions must be received by Wednesday to be counted.
