Faith Formation Mass Attendance

This form is to be used when you visit another parish for Mass and you are unable to sign-in to St. William's using the QR Codes. You will also be asked to upload a photo of your kids with with the parish bulletin or in front of the Catholic church you visited. Submissions must be received by Wednesday to be counted for the appropriate weekend.
