PARTICIPATION ACKNOWLEDGEMENT
I understand that participation as a scare actor at Collins Farm involves an outdoor haunted attraction conducted in and around a corn maze, including during nighttime hours.

I understand that participation may involve walking, standing, crouching, moving through cornfields, performing in darkness or low-light conditions, wearing costumes or masks, interacting with frightened guests, exposure to loud noises, theatrical lighting, fog or atmospheric effects, changing weather conditions, insects, mud, wet ground, uneven terrain, roots, rocks, corn stalks, farm equipment, props, and other conditions normally associated with an outdoor farm attraction.

I understand that these conditions involve inherent risks, including but not limited to slips, trips, falls, strains, sprains, cuts, bruises, collisions, exposure to weather, and unexpected reactions from guests.

I understand that this is an unpaid volunteer position.

ASSUMPTION OF RISK
I voluntarily choose to participate in the Collins Farm haunted attraction and understand that participation may involve risks that cannot be completely eliminated.

I agree to exercise reasonable care, follow all instructions given by Collins Farm staff, remain within assigned acting areas, and immediately report unsafe conditions, injuries, damaged equipment, or inappropriate guest behavior.

To the extent permitted by law, I knowingly and voluntarily assume the ordinary and inherent risks associated with my participation.

ACTOR CONDUCT
I understand and agree that while participating as a scare actor:

I will follow the instructions of Collins Farm management and designated supervisors.
I will remain in my assigned area unless given permission to move elsewhere.
I will not intentionally strike, grab, shove, restrain, trip, chase into an unsafe area, or otherwise intentionally make physical contact with guests.
I will not use weapons, dangerous props, pyrotechnics, or unauthorized equipment.
I will not intentionally damage corn, props, costumes, signs, buildings, equipment, or other property.
I will not participate while under the influence of alcohol, marijuana, illegal drugs, or any substance that impairs my judgment or ability to perform safely.
I will treat guests, other actors, and Collins Farm staff respectfully.
I understand that guests may scream, swear, run, become frightened, or react unpredictably.
I will immediately seek assistance from Collins Farm staff if a guest becomes aggressive, threatening, injured, or otherwise creates an unsafe situation.
I understand that Collins Farm may change my role or acting location or end my participation at any time for safety, operational, or behavioral reasons.

PERSONAL RESPONSIBILITY
I agree to tell Collins Farm staff before participating if there is any limitation or circumstance that should be considered when assigning my role, such as needing a stationary position or being unable to safely perform particular physical activities.

I understand that I am responsible for using reasonable judgment and for notifying a supervisor if I believe an assignment or condition is unsafe for me.

EMERGENCY MEDICAL AUTHORIZATION
If I am injured or experience a medical emergency while participating and I am unable to make decisions for myself, I authorize Collins Farm representatives to contact emergency services and obtain reasonably necessary emergency assistance.

I understand that Collins Farm personnel are not responsible for providing professional medical care and that emergency medical services may be contacted when appropriate.

RELEASE AND WAIVER
To the fullest extent permitted by applicable law, I release and hold harmless Collins Farm, Collins Farm LLC, its owners, employees, representatives, agents, and property owners from claims arising from the ordinary and inherent risks of voluntarily participating as a haunted attraction actor.

This release is not intended to waive any rights that cannot legally be waived under applicable law.
