Mizuta & Associates Physical Therapy will assess fees for the following conditions:
a. Cancellation Fee (Less than 24-hour notice): $40.00*
b. No Show Fee: $100.00
c. Returned Check Fee: $25.00
All credit/debit card transactions will have an additional 3% processing fee.
Patients who consecutively cancel will be subject to same day scheduling.
