I understand that payment is to be made via direct deposit upon invoice. I understand payments will be due on the 1st of the month. I understand a $10 late fee per week will be due if my payment fails to process and my child may be asked to sit out of classes. I understand that if my child does not attend classes I will still be charged for them as I am paying for a position in that class. There will be no makeup classes provided. I also understand the timetable is currently in its tentative stage and is subject to changes. Full Force Dance and Cheer will only send information to the responsible person & nominated parties on the Full Force Dance and Cheer Registration Form. If contacted for information regarding an athlete by another party Full Force Dance and Cheer requires permission in writing from the Responsible Person who is responsible for enrolment.

My child WILL be participating in the end of year concert on the 6th of December at Plumpton High School. I am aware I will be invoiced and need to pay for a costume for the end of year concert. Each class requires a different costume
My Child WILL NOT be participating in the end of year concert.
I understand there will be a $25 registration fee due at the time of registration, and that I am responsible for all payments being the guardian to register my child at Full Force Dance and Cheer. Cancellation of my athletes placement will require 1 month / 30 days notice and I will be charged accordingly.
