Better Strides Equine Bodywork Assumption of Risk and Waiver of Liability

(owner name) am allowing my animal,

to receive bodywork (massage, red light therapy, PEMF, TENs therapy, kinesio taping) from Brianna York (practitioner) with Better Strides Equine Bodywork I understand that bodywork is not a substitute for medical treatment or medications and that it is recommended that I work with my veterinarian to diagnose and treat any medical conditions that my animal may have. I understand that bodywork sessions are for supportive purposes only: general relaxation, relief from muscular tension, improvement of circulation, reduction of inflammation, pain relief, and facilitation of natural body processes.

Iunderstand that my practitioner is not a medical professional and cannot diagnose illness or disease or prescribe medications. I understand that any information exchanged by my practitioner is educational in nature, and is not diagnostically prescriptive and should be used at my own discretion.

I understand that the above-listed therapies will not cause illness or injury, such as colic, abscess, or other medical conditions. However, if such a condition is in the process of presenting, treatment may make them present more quickly. I understand that rarely, animal users can experience muscle soreness, stiffness, changes in fecal output, hives or skin changes, mood/behavioral changes, and should seek veterinary attention accordingly.

By signing this release, I hereby fully and forever waive, indemnify, hold harmless and release Better Strides Equine Bodywork, its affiliates, and all of its respective members from any and all claims, damages, demands, rights of action, or causes of action, presentor future, known or unknown, anticipated or unanticipated, resulting from services provided to me from my use of the services of Better Strides Equine Bodywork. In case of an emergency during the treatment, I understand that the Practitioner will immediately try to contact me. If I cannot be reached, the Practitioner is authorized to seek immediate veterinary care. I agree to reimburse Better Strides Equine Bodywork, for any additional costs incurred as a result. By signing this release, I declare that I am signing of my own free will. By signing this release, I certify that I am eighteen (18) years of age or older or have provided the consent of my parent(s) or guardian to the Practitioner, Brianna York, Better Strides Equine Bodywork. I further agree that I will be liable to Better Strides Equine Bodywork, for any attorney fees or costs incurred by Better Strides Equine Bodywork, to enforce this Assumption of Risk and Waiver of Liability. Horse owner/guardian agrees to release Brianna York of Better Strides Equine Bodywork from any loss, damage, liability or injury arising out of, or resulting from, participation in equine massage therapy/acupressure/kinesiotaping therapy/ TEN: treatment/red light therapy, including any negligent acts or omissions.

Better Strides Equine Bodywork

Horse owner/guardian has been notified that payment must be made upon arrival of the equine service(s) that has been requested. If payment is not made at this time, payment will be subjected to a $5/week fee on top of the treatment fee owed. There are absolutely no exceptions to this policy. Payment is accepted in the form of CASH, SQUARE, or Paypal to @appalapalooza@yahoo.com Any NSF checks returned will be charged a $25 fee on top of the treatment fee and due immediately in CASH, SQUARE, or via PAYPAL. Horse owner/guardian has been notified that Brianna York of Better Strides Equine Bodywork reserves the right to maintain a 24-hour cancellation policy. Appointments must be canceled within 24 hours of the scheduled meeting, otherwise, clients will be charged 50% of the scheduled appointment fee. No exceptions will be made to this policy. Horse owner/guardian understand they are to be present at every scheduled appointment or have a responsible, horse-knowledgeable person represent them during the appointment. If horse owner/guardian does not attend scheduled appointment, clients will be charged 50% of the scheduled appointment fee. No exceptions will be made to this policy.

I have carefully read this Assumption of Risk and Waiver of Liability and fully understand that it is a waiver and release of any and all liability without expiration. I fully understand its contents, and voluntarily agree to the terms and conditions stated above. I acknowledge that I have had ample opportunity to read this entire document, ask any questions I might have, and consult with a legal counsel if I wish. If any portion of this agreement is held invalid, the remainder of the agreement shall remain in full force and effect.
