Application Checklist
All Applicants

If the applicant lives out of state(CA), please contact us first. daomadmissions@fivebranches.edu

To be considered for admission to Five Branches University, your application package must include the following items. For more details, refer to the Admissions section of the catalog or website, www.fivebranches.edu

These are the initially required documents. Please have the following documents ready when you begin filling out this application form:

1. Application for Admission (completed in full)
2. Digital Photograph- passport style
3. Statement of Purpose
4. Resume
5. Non-refundable Application Fee: $100 (Payable via PayPal using Visa, Mastercard, Discover, or American Express only)

The following required items will go through a document upload process and can be completed now or later:

7. Copy of Master’s or professional doctorate level diploma in TCM, Oriental Medicine, or Acupuncture
8. Copies of California L.Ac. Pocket License, State License, or National Certifications (required only if applicable)
9. Copy of Valid Malpractice Insurance With Five Branches University Listed as Additional Insured (required only if applicable)

10. Two Letters of Recommendation sent via email to daomadmissions@fivebranches.edu
11. Official transcripts for all AOM and healthcare-related college/university coursework, enclosed in university-sealed envelopes and mailed directly to Five Branches University (Required, but exceptions can be made, such as when applicants are completing their program and the transcript is not yet available at the time of application.)
Transfer Applicants(Transferring from a TCM/acupuncture school)

Applicants who wish to transfer from another TCM college must submit the following materials (in addition to the materials indicated for All Applicants):

1. Cover letter stating your reasons for seeking transfer and the courses you wish to transfer
2. An additional letter of recommendation from a previous TCM/Acupuncture instructor, located at the DAOM school you are attending.
3. Catalog from your previous DAOM program, stating its approval agencies and accreditation.
4. Syllabus outlining the content for each course you wish to transfer.
International Applicants

International applicants applying for an F-I student visa through Five Branches University must submit the following documents (in addition to the materials indicated for All Applicants):

1. A photocopy of the first page of your passport (also of dependents, if applicable)
2. Transcript evaluation by a member of NACES
3. English proficiency test report with score
4. International Applicant Financial Statement
5. Proof of Health Insurance (We recommend purchasing health insurance in your home country due to high U.S. costs, with a minimum coverage of $250,000 per incident and a deductible under $500 per person. Obtain medical and dental records before arriving.)
International Applicants (Currently studying in the U.S)

International applicants who are currently on an F-1 student visa and enrolled at another college must submit the following
documents (in addition to the materials indicated for All Applicants and International Applicants):

1. A photocopy of your visa, contained in your passport (also of dependents, if applicable)
2. A photocopy of your latest I-20 form

Federal financial aid and Graduate PLUS loans are available for qualifying students. To apply, go to www.fafsa.gov and enter the Five Branches school code: 031313.
